Iran sets the full basic health insurance premium at 4.81 million toman a year for 1405, with deciles one to five covered free

28 July 2026 | 3 months ago

The Iran Health Insurance Organisation set out new details on Tuesday, 28 July 2026, of how income deciles qualify for basic health insurance cover. Under the announcement, the full basic health insurance premium for 1405 (the Iranian year running to March 2027) has been set at 4,810,320 toman per person per year.

Mohammad Gholamnejad, head of the organisation's Tehran province office, said people in the first to fifth income deciles are covered free for one year, automatically and without needing to apply in person, and that patients flagged in the fund for special and hard to treat illnesses are exempt from the premium across all deciles. He said more than 80 percent of the organisation's insured members currently pay no premium at all.

The government pays part of the premium for the sixth to ninth deciles. The sixth decile, with 60 percent state support, pays 1,924,128 toman a year; the seventh, with 50 percent support, pays 2,405,160 toman; the eighth, with 40 percent support, pays 2,886,192 toman; and the ninth, with 30 percent support, pays 3,367,224 toman. The tenth decile receives no state support and pays the full amount.
